Choosing an HMS: a buyer’s checklist for Indian facilities

From single-doctor clinics to multi-branch chains, the right HMS depends on your care setting. A practical checklist covering modules, compliance, AI and total cost.

The “best” Hospital Management System doesn’t exist in the abstract — only the best one for your facility. A single-doctor clinic, a 200-bed hospital and a diagnostic-lab chain have genuinely different needs. Use this checklist to cut through the demos.

1. Does it fit your care setting?

A chemist shouldn’t have to navigate around OT scheduling; a hospital shouldn’t be missing a blood bank. The right HMS scopes itself to your segment — and lets you add capabilities as you grow, without migrating to a different product.

2. Is it genuinely India-first on compliance?

  • ABDM / ABHA linking and FHIR R4 exchange
  • DPDP consent handling
  • PCPNDT (Form F) for facilities doing ultrasound
  • Drug schedules (H / H1 / X) and a controlled-drug register
  • GST with HSN, and biomedical waste registers
Generic global HMS products almost never handle PCPNDT, drug schedules or ABDM out of the box. In India, that gap is not a detail — it’s a compliance liability.

3. Is the AI safe and useful?

Look past the demo sparkle. Ask two questions: is the AI grounded in the patient’s real data, and is it advisory-only with a human approving before anything saves? If the answer to either is no, be cautious.

4. What’s the real total cost?

Count licence sprawl, integration fees and the staff hours lost to double data-entry and reconciliation across disconnected tools. Consolidation onto one platform often pays for itself here — and the savings compound with every branch you add.

Ask any vendor to run the demo on your workflow, not theirs: register a patient, take them through a consult, an order, a bill and a discharge — and watch how many separate systems the data has to cross. The fewer, the better.
